Service program
The Integrated Team Care (ITC) Program aims to increase access to primary health care services for Aboriginal and Torres Strait Islander people. The program offers care coordination support for people with a chronic condition, and general support to access health services through outreach workers. Can assist with cost of medication, transport to and from appointments and care coordination.

Referral pathways
For Care Coordination, patients need a 715, GPMP or GP referral. For Indigenous Outreach Worker, referrals can be made via self-referral, GP or Aboriginal Health Worker

Intake capacity and limitations
Care Coordinator and Supplementary Services require that the person identify as Aboriginal and/or Torres Strait Islander, have a current 715 Health check and GP Management Plan, have a referral from GP and have a diagnosed chronic condition. For access to an Indigenous Outreach Worker the person must identify as Aboriginal and/or Torres Strait Islander and require assistance to attend a Medical Appointment.
