Psychosocial Support offers peer support for people who are diagnosed with a severe and complex mental health condition who are currently being managed in the primary care setting via a GP and/or psychiatrist. People who are eligible will receive psychosocial services through a range of non-clinical supports which will be provided as individual or group modality. These services build ability and skills to assist people to manage their mental health, improve relationships with family and others, and increase social and economic participation.
Model of service delivery
sychosocial Support will be delivered through: face-to-face sessions; telephone or online to individuals either in private or general practice settings; and/or community locations or via home visits (where risk assessed as safe and appropriate).
Eligibility criteria (including age)
ged 18 and older, who: are diagnosed with a severe and complex mental illness; have mental health issues that are significantly impacting on their social, personal and work life; would benefit from specialised psychosocial support at particulat points in time to assist them build capacity for daily living; are not assisted by the NDIS; and are currently under the care of a GP or psychiatrist.
Referral pathways
GPs, psychiatrists, self-referral, or other community or NGO type organisation.
